The period during which the transplanted hairs fall out and then grow back after a hairline hair transplant surgery, aimed at correcting wide or high foreheads or M-shaped hairlines, is commonly referred to as the “telogen phase.” During this phase, the empty-looking hairline can cause discomfort in daily life.

How long does this telogen phase last? While it varies from person to person, the transplanted hairs typically start to fall out gradually from 2 to 4 weeks post-surgery and begin to regrow after about 3 to 6 months. However, the duration of the telogen phase is not the same for everyone. The length and amount of the telogen phase can vary depending on the know-how used to minimize follicle damage during the hair transplant process. Patients at our clinic often report that their telogen phase was much shorter or that they experienced less hair loss compared to their friends who had surgeries at other clinics.

In fact, a survey of our patients who underwent hairline correction surgery showed a much lower hair loss rate during the telogen phase (30-60% loss rate) compared to the usual rate (70-90% loss rate). Female patients were able to resume their normal activities without covering the surgery site approximately 4 months post-surgery, while male patients took about 5 months.

The reason for the shortened telogen phase is due to our clinic’s unique ‘Dual Slit & Forceps’ technique. The slit method involves creating tiny holes in the hairline for the follicles to be transplanted into, and forceps are used to carefully place each follicle into these slits. For a natural-looking hairline, we do not use an implanter but rely solely on the slit & forceps technique to ensure the higher survival rate of the fine hairs.

However, in some clinics that use the slit & forceps method, the surgeon creates the slits, and the nurse performs the follicle transplantation. If the follicles are thicker or larger than the slit size, they can be damaged by folding or crumpling during the transplantation process. This is especially problematic for thin hairs, which have a lower survival rate and may not grow if not properly transplanted.
At our clinic, we insert a second (dual) slit just before transplantation to ensure that there is no friction when the follicles are placed into the holes. This adjustment significantly contributes to the shortened telogen phase and the high survival rate of thin hairs.
